OSF OnCall is transforming Medicaid care delivery in Illinois by improving quality, increasing access and addressing health disparities by bringing health care to patients in distressed communities in a new and innovative way. Using Bluetooth devices and digital platforms, outreach, and engagement, Medicaid patients may enroll in remote patient monitoring programs for chronic disease management, health literacy and wellness or pregnancy and postpartum care. A key component of enrollment in each program is the opportunity for connecting with a Digital Health Navigator and completing an assessment for Social Drivers of Health.
