New users
After each visit to SIU Medicine, you'll receive an invitation to join MyHealth@SIU Medicine via email. Follow the instructions provided. Your birth year is your invite code.
Miss the email? You can stop by your clinician's office or request a new account. If you are also a patient at Memorial Care and already use their patient portal, simply add SIU Medicine as a connected organization.

Manage your family's health
The proxy feature gives you access to the records of your minor children, birth through age 11, and dependent adults or adults for which you may have legal guardianship, power of attorney, etc. SIU Medicine must have any legal documents on file to allow proxy account access. Request a proxy account at the patient's SIU Medicine provider's office.
Provide your email during registration to receive an invitation with a link. Follow the instructions to create an account. Your invite code is your birth year.
Yes, the portal uses encryption and other security measures. Use a strong, confidential password for added protection.
Go to myhealthsiumed.org or use the FollowMyHealth App. Enter your username and password, then click “Sign In.
Click “Forgot Password” on the login page, enter your username, and check your email for a verification code.
Yes. Go to My Account > My Info to update contact details, address, and insurance.
Portal info is only uploaded at account creation. Ensure updates are made directly in the office.
Log in and go to the “My Health” section to view test results.
